Ahpra receives and manages concerns on the Board&#8217;s behalf, but the Board makes the decisions. In <strong>New South Wales<\/strong> concerns are handled by the <strong>Pharmacy Council of NSW<\/strong> (with the Health Care Complaints Commission), and in <strong>Queensland<\/strong> they go first to the <strong>Office of the Health Ombudsman<\/strong>, which decides what to refer on.<\/p>\r\n\r\n    <h2>What triggers a notification about a pharmacist?<\/h2>\r\n    <p>A notification is a concern about a pharmacist&#8217;s health, conduct or performance. For pharmacists the common triggers are distinctive: dispensing and labelling errors, breaches of drugs-and-poisons legislation, medication misadventure, supply or record-keeping problems, privacy breaches, and conduct concerns. Anyone can raise a notification, and pharmacists, employers and education providers have a duty to make a <strong>mandatory notification<\/strong> where they reasonably believe notifiable conduct has occurred.<\/p>\r\n\r\n    <h2>What happens after a notification is made?<\/h2>\r\n    <p>The concern is first <strong>assessed<\/strong> \u2014 usually within around 60 days \u2014 and you are generally given the chance to respond. From there the Board may take no further action, ask for more information, refer the matter to a health or performance pathway, open a formal <strong>investigation<\/strong>, or, only where the public is at serious and current risk, take <strong>immediate action<\/strong> on your registration.<\/p>\r\n    <aside class=\"note\"><p><span class=\"note-h\">Most matters do not restrict your registration<\/span>A large share of notifications close after assessment. Suspension or conditions are reserved for genuine, current risks to the public.<\/p><\/aside>\r\n\r\n    <h2>Possible outcomes<\/h2>\r\n    <p>Outcomes run from no further action, through a caution, undertaking or conditions on registration, to referral to a panel or tribunal in the most serious cases.
